connective tissue 결함 → abdomen aneurysm이 가장 흔함

                                     Transforming growth factor - bata↑, fibrillin-1 ↓

                                     dilatation of the aortic ring

                                     Generalized disorder

                                     Aortic dissection, AR, MVP

 

■ MVP

Mid systolic click = late systolic

순간적인 일탈로 MV diameter increased

MV에 prolapse생기면 AV나 TV에도 prolapse 생길가능성 ↑

MR동반이 많고 , MR과 동일하게 LVVO

※ flail 구분법 : valve tip방향의 차이

 - prolapse : LV방향

 - flail : LA방향

 

■ Aorta Dissection

 - Recognition of intimal flap within aorta

 - intima tear of aortic wall

 - pleural effusion , pericardial effusion

 - AR발생

 - dissection의 분류 : Stanford type A : ascending aorta포함 (Debakey Ⅰ,Ⅱ로 나뉨)

                                  Stanford type B : descending aorta쪽 dissection (Debakey Ⅲ)

intramural gematoma ← penetrating ulcer ← dissection

                                   ← vasa vasorum(vessel of vessel)의 rupture

 

증상

 - Chest pain that radiated to the back

 - systemic hypertension + LV hypertrophy (aorta에 부담 多, dissection 발생)

 - widening of the superior mediastinum

 

 

 

descending thoracic aorta를 볼수있는 view : 2champer apical view

 - thoracic aorta aneurysm관련 질환 : Syphilis

 

Bicuspid Aortic valve : Hammock appearance aortic valve leaflet

                                    post stenotic aortic root dilatation

 

supravalvular aortic stenosis

LVVO (X), LVPO (O)

AS+AS동시에 있는 경우 MPG로 측정.
